- To fulfil or meet the reason for which the information is provided. For example, to respond to your questions or communications regarding membership in an HCM-affiliated practice, Services, pricing, or affiliated physicians. If you become a member in an HCM-affiliated practice, we may use your personal information to process your payment, provide membership and renewal information and to support and facilitate your receipt of HCM Services or programs.
- To publish content on the Website when, under some circumstances, HCM publishes testimonial comments or enables public participation forum by Users on the Website.
- To personalize your Website experience and to deliver content and service offerings relevant to your interests, including targeted offers and ads through our Website, third-party sites, or via email or text message (with your consent, where required by law).

IP ADDRESSES, COOKIES AND OTHER TECHNOLOGY
HCM maintains internal logs for Internal Protocol (IP) addresses. An IP address is a number that is automatically assigned to your computer each time you access the Internet.
