No. Insurance companies do not cover immigration physicals due to non-medical necessity, therefore, it cannot be submitted to your insurance company.

• The cost of an adult physical (14 years and 1 day and up) is $400 plus the cost of vaccines, if needed.
• The cost of a child physical (2 years and 1 day-14 years) is $300 plus the cost of vaccines, if needed.
• The cost of a newborn physical (0 years – 2 years) is $300 plus the cost vaccines, if needed.

• Valid, government issued picture ID such as a driver’s license or passport (preferred).
• Proof of prior vaccinations.
• Please bring an interpreter if needed.

The cost of the Immigration Medical Physical Examination is $475 (not including vaccinations) and includes the following:

• Basic Immigration Medical Physical Examination $250
• IGRA Test $150
• Gonorrhea Test $55
• Syphilis Test $45
• Vaccination status verification
• Copies of your laboratory results
• Certified and Sealed I-693 Form

Additional tests listed below are not included in the price of the exam but are offered for an additional fee. This will add to the cost of the exam. Payment is by cash or credit card. We do not accept major medical insurance for all services required for the I-693 Immigration form.

Yes. You need to bring documentation for every diagnosed medical condition and all prescribed medications that you may have, including but not limited to pregnancy, diabetes, cancer, thyroid conditions, etc.

For pregnancy, we need a document from your OB stating that you are pregnant and permission/no permission to receive an X ray (if necessary) and vaccinations (if necessary).

Yes, your blood will be tested for syphilis, tuberculosis gold and gonorrhoea.

No. Fasting is not required for lab work.

No, however, you do have to bring a photo ID for identification purposes.

Yes. If you cancel your appointment within 24 hours of its scheduled time, you will be charged $50.

The typical turnaround time is 5-7 business days.

The physical must be completed within 2 years of your application, however, due to recent legislation, the physician’s signature is only valid for 90 days from the date of your evaluation.

You may schedule another appointment or walk in, to get the form re-issued and with a new signature. The cost for this is $50.00.

We suggest that you arrive at least 30 minutes prior to your appointment time in order to complete the immigration packet.

If you are unable to read and write proficiently in English, you are required to bring a translator to your appointment to aid you in completing the immigration packet.
