What Is the Purpose of an Immigration Physical?

The purpose of an immigration physical is to confirm that you meet the medical requirements for permanent residency in the United States. The examination must be completed by a Civil Surgeon designated by U.S. Citizenship and Immigration Services (USCIS).

At Houston Concierge Medicine, Dr. Gregory Burzynski is a certified Civil Surgeon and can perform the required medical examination, review your vaccination history, coordinate required testing, and complete the necessary Form I-693 documentation.

Immigration physicals are self-pay and are not covered by medical insurance.

What to Expect

Requirements for Immigration Physicals

Your immigration medical examination includes several required evaluations and tests based on USCIS guidelines, your age, and your vaccination history.

Medical & Mental Status Evaluation A basic immigration physical examination and mental status evaluation are completed as part of the required medical exam.

Syphilis Blood Test Adults ages 18–44 are required to complete a serologic blood test for syphilis (RPR).

Gonorrhea Testing Adults ages 18–24 are required to complete testing for Neisseria gonorrhoeae.

Tuberculosis Testing Applicants age 2 and older are required to complete an IGRA tuberculosis blood test.

Vaccination Review Bring proof of your previous vaccinations or available blood titer results so your required immunization status can be reviewed.
Please bring all vaccination records to your appointment. Dr. Burzynski will review your vaccination history to determine whether you meet the required immunization guidelines. Additional vaccinations are not included in the basic exam price but are available through the office for an additional fee.

How this guide works

This is an age-based educational guide for U.S. adjustment-of-status applicants. It does not determine whether you are fully vaccinated, qualify for a waiver, or are ready to submit Form I-693. It does not calculate vaccination dose schedules.

It uses the CDC's age-specific vaccination requirements for civil surgeons, which differ from routine vaccination recommendations. The Civil Surgeon makes the final assessment. Seasonal influenza vaccine requirements depend on availability at your appointment. Rotavirus vaccination has strict infant-age limits.

Vaccines outside the CDC immigration-specific list, including HPV, shingles, and COVID-19, are not shown in this guide.

Once your immigration medical examination and required testing are complete, your Civil Surgeon will finalize the medical documentation required for your immigration application.

Form I-693 Is Completed Dr. Burzynski will complete and certify Form I-693 using your medical examination, required test results, and vaccination history.

Your Documentation Is Sealed Your completed immigration medical documentation will be provided according to current USCIS submission requirements.

Keep Your Copies Copies of applicable laboratory results and other records may be provided for your personal records after they have been reviewed with you.
Important Do not open any sealed USCIS envelope provided to you for submission.

No. Insurance companies do not cover immigration physicals due to non-medical necessity, therefore, it cannot be submitted to your insurance company.

The cost of an adult physical (14 years and 1 day and up) is $400 plus the cost of vaccines, if needed.
The cost of a child physical (2 years and 1 day–14 years) is $300 plus the cost of vaccines, if needed.
The cost of a newborn physical (0–2 years) is $300 plus the cost of vaccines, if needed.
Valid, government-issued picture ID such as a driver’s license or passport (preferred).
Proof of prior vaccinations.
Please bring an interpreter if needed.

The cost of the Immigration Medical Physical Examination is $475 (not including vaccinations) and includes the following:

Yes. You need to bring documentation for every diagnosed medical condition and all prescribed medications that you may have, including but not limited to pregnancy, diabetes, cancer, thyroid conditions, etc.

For pregnancy, we need a document from your OB stating that you are pregnant and permission/no permission to receive an X ray (if necessary) and vaccinations (if necessary).

Yes, your blood will be tested for syphilis, tuberculosis gold and gonorrhoea.

No. Fasting is not required for lab work.

No, however, you do have to bring a photo ID for identification purposes.

Yes. If you cancel your appointment within 24 hours of its scheduled time, you will be charged $50.

The typical turnaround time is 5-7 business days.

The physical must be completed within 2 years of your application, however, due to recent legislation, the physician’s signature is only valid for 90 days from the date of your evaluation.

You may schedule another appointment or walk in, to get the form re-issued and with a new signature. The cost for this is $50.00.

We suggest that you arrive at least 30 minutes prior to your appointment time in order to complete the immigration packet.

If you are unable to read and write proficiently in English, you are required to bring a translator to your appointment to aid you in completing the immigration packet.

Our aim is to simplify the process for you as much as possible. If you have more than one person who requires an exam, kindly inform us so that we may book you in for back-to-back appointments. During your exam, all of your medical concerns will be addressed. However, if you have any additional questions or wish to book an appointment, please feel free to give us a call.

We strive to complete your exam for you as quickly and affordably as possible. It's important to note that we do not accept major medical insurance for all services required for the I-693 Immigration form. This includes the immigration medical exam, labs, and immunizations necessary for documentation completed at the time of service.
