Houston Concierge Medicine

Welcome to Houston Concierge Medicine. By enrolling in a membership plan, scheduling services, or utilizing services provided by the Practice, you acknowledge and agree to the following Membership, Financial & Cancellation Policy.
Concierge Membership Services
Houston Concierge Medicine provides concierge healthcare services based on the selected membership plan. Services may include, but are not limited to:

Annual Physical Examinations

Laboratory Testing
-
Virtual Visits
-
In-Person Visits
-
House Calls
-
Preventive & Wellness Services
-
Concierge Care Coordination
-
Specialist Coordination
-
Prescription Management Support
Certain services may involve coordination with third-party laboratories, imaging facilities, pharmacies, hospitals, or specialists as medically appropriate.
Membership services are not health insurance and do not replace traditional insurance coverage.

All memberships require a twelve (12) month commitment unless otherwise expressly stated in writing by Houston Concierge Medicine.
The initial payment is due at the time of enrollment. Monthly memberships automatically renew and recurring payments will be charged on the same billing date each month using the payment method provided by the member.
By enrolling in membership services, the member authorizes Houston Concierge Medicine to charge all applicable fees, balances, taxes, late fees, and approved charges to the payment method on file.
Membership Term & Cancellation
Standard Membership Commitment
All Silver, Gold, and Platinum memberships are subject to a twelve (12) month contractual commitment.
Following completion of the initial membership term, memberships will continue on a month-to-month basis unless canceled with a minimum of thirty (30) days written notice.
Early Termination
If a member elects to terminate membership prior to completion of the initial commitment period, the member remains financially responsible for the remaining balance of the agreement term.
Houston Concierge Medicine reserves the right to suspend or terminate membership services immediately under the following circumstances:

Breach of this Agreement

Abusive, threatening, or inappropriate behavior toward staff

Fraudulent or misleading information provided during enrollment or treatment

Non-payment or declined recurring payments
-
Medical circumstances preventing continuation of care

Closure, dissolution, or discontinuation of Practice operations
Termination of membership does not eliminate responsibility for outstanding balances incurred prior to cancellation or termination.
Short-Term or Promotional Memberships
If Houston Concierge Medicine offers a short-term, promotional, or six (6) month membership option, cancellation prior to completion of the minimum term will result in responsibility for the remaining balance due under the agreement.
Unless otherwise stated in writing, all promotional pricing is non-transferable and non-refundable.
Appointment Cancellation Policy
Office Visits & Virtual Visits
A minimum of twenty-four (24) hours notice is required to cancel or reschedule appointments.
Failure to provide proper notice may result in a missed appointment fee.
Repeated no-shows or late cancellations may result in suspension or termination of scheduling privileges.
House Call Services
House Call appointments require a minimum of twenty-four (24) hours notice for cancellation or rescheduling.
Appointments scheduled within twenty-four (24) hours of the visit time are considered final and non-cancelable.
Failure to provide proper notice may result in charges up to the full amount of the scheduled service.
Group Appointments
Group appointments involving two (2) or more individuals are subject to the same cancellation requirements.
Absent participants without proper notice may still be billed for the reserved service time.
For groups of four (4) or more individuals, cancellations after confirmation of staffing, scheduling, or nurse assignment may incur a $50 cancellation fee per participant.
Executive Physicals
Full payment is required at the time of booking to reserve an Executive Physical appointment.
A minimum of twenty-four (24) hours notice is required for cancellations or rescheduling.
Failure to provide proper notice, failure to attend the appointment, or failure to comply with required pre-visit preparation instructions may result in forfeiture of all or part of the payment.
Additional specialty diagnostics, laboratory testing, imaging, or add-on services are non-refundable once ordered, scheduled, processed, or completed.
Approved refunds, if applicable, will be issued less non-refundable merchant processing fees and returned to the original payment method within approximately 5–10 business days.
Prescription Medications & Medical Products
Prescription medications, compounded medications, supplements, injectable therapies, and pharmacy-dispensed products are non-returnable and non-refundable once processed, dispensed, shipped, or administered.
This includes, but is not limited to:

TRT therapies
-
Enclomiphene
-
Medical Weight Loss medications
-
Injectable medications
-
Customized compounded therapies
Financial Responsibility
The member/patient acknowledges full financial responsibility for all services rendered by Houston Concierge Medicine that are not covered by insurance or otherwise prepaid through membership.
Declined or failed payment methods may result in suspension of membership services until balances are resolved.
Houston Concierge Medicine reserves the right to utilize collection efforts or third-party recovery services for unpaid balances where legally permitted.
Refund Policy
Except where otherwise required by law, membership fees are non-refundable.
Refunds or account credits may be issued solely at the discretion of Houston Concierge Medicine in situations where services cannot be reasonably performed due to circumstances outside of the patient’s control.
Patient Conduct & Misrepresentation
Houston Concierge Medicine reserves the right to refuse or discontinue services if a patient:

Provides inaccurate, incomplete, or misleading information

Misrepresents medical history or eligibility
-
Appears impaired by drugs or alcohol
-
Engages in disruptive, threatening, abusive, or inappropriate conduct
In such cases, scheduled appointment fees and applicable service charges may still apply.
Limitation of Liability
Houston Concierge Medicine shall not be liable for delays, interruptions, or inability to provide services resulting from events beyond reasonable control, including but not limited to natural disasters, public health emergencies, technology failures, third-party vendor disruptions, or governmental actions.
Governing Law
This Policy and any disputes arising from membership services shall be governed and interpreted in accordance with the laws of the State of Texas.
Any legal proceedings arising from these services shall be subject to the jurisdiction of the courts located within the State of Texas.
Severability
If any provision of this Policy is determined to be invalid or unenforceable under applicable law, the remaining provisions shall remain in full force and effect.
Entire Agreement
This Policy constitutes the entire understanding regarding membership billing, cancellations, financial obligations, and related services between Houston Concierge Medicine and the patient/member unless otherwise modified in writing by the Practice.
Policy Updates
Houston Concierge Medicine reserves the right to update or modify this Policy at any time without prior notice. Updated versions will be posted on the Practice website with the revised effective date.
