An Ongoing Relationship With Your Physician

Our Silver, Gold, and Platinum memberships include ongoing primary care visits, an annual physical, lab testing, and a concierge care coordinator. Additional testing and access benefits vary by membership level.

Appointments & Payment

Know Your Options Before You Enroll

In-office and eligible virtual visits are available, with access benefits determined by your membership. Our memberships are 12-month plans with monthly or annual payment options. HCM does not accept health insurance.
