This AI tool will assist our medical staff by transcribing and documenting information discussed during your appointment. Please be assured that all information recorded will be handled with the highest level of confidentiality and in compliance with privacy regulations, including HIPAA.

By signing this consent form, you acknowledge and consent to the use of AI scribing in our medical office as part of your care. If you have any concerns or questions regarding the use of AI or how your information will be managed, please feel free to ask our staff.

Right to Revoke: You will have the right to revoke this Consent at any time by giving us written notice of your revocation submitted to the Contact Person listed above. Please understand that revocation of this Consent will not affect any action we took in reliance on this Consent before we received your revocation, and that we may decline to treat you or to continue treating you if you revoke this Consent.
