Your Insurance Policy Is a Contract Between You and Your Carrier
Coverage, benefits, limitations, deductibles, copays, and network participation can vary by plan and may change over time. Verify your benefits directly with your insurance company. The phone number on the back of your insurance card is typically the best place to begin.

Confirm Network Participation
Contact your insurance company to confirm that the physician you are seeing is considered an in-network contracted provider under your specific plan.

Verify Your Primary Care Physician
If you have an HMO plan, confirm that the primary care physician listed with your insurance company is the provider you will be seeing.
03 / CONFIRM
Verify Benefits Directly
Do not rely solely on provider directories or office staff to confirm participation. Verify your benefits and network status directly with an insurance representative.
04 / BRING
Bring Your Current Insurance Card
Please bring your current insurance identification card to each office visit so proof of coverage and necessary claim information can be documented.

Understanding Patient Responsibility
It is ultimately the patient's responsibility to understand the benefits and limitations of their insurance policy. Houston Concierge Medicine and its physicians do not establish the terms of your insurance contract.
Charges for services that are not covered by your insurance company may become your responsibility. Some services may also be offered on an out-of-pocket basis, and applicable fees will be communicated in advance when appropriate.
Failure to provide current proof of coverage may require payment at the time of service until the necessary insurance information is available for claim submission.
Self-Pay Patients
No Insurance? You Can Still Schedule Care.
Patients without insurance are welcome to schedule an appointment. Self-pay fees are based on the time and expertise required to evaluate and treat your concerns.
